Nommé Premier ministre par décret du 8 janvier 1959, Michel Debré est le premier titulaire d’une fonction créée par l’article 21 de la Constitution de 1958. Le cabinet de Michel Debré, d'abord restreint, s'étoffe progressivement par plusieurs arrêtés qui s'étalent sur tout le mois de janvier 1959 mais qui retiennent comme date d'effet le 9 janvier 1959. Ce cabinet connait par la suite diverses modifications jusqu'en janvier 1962.
Pierre Racine, chef de la mission d'études du secrétariat général pour les Affaires algériennes depuis juillet 1958, conseiller d’État, est nommé directeur du cabinet en janvier 1959 et conserve ces fonctions jusqu’au départ du Premier ministre en avril 1962. Yves Guéna, ancien résistant, maître des requêtes au Conseil d'Etat, ancien directeur du cabinet de Michel Debré lorsqu’il était ministre de la Justice (juin 1958-janvier 1959), est désigné directeur-adjoint du cabinet. Deux chefs de cabinet sont nommés : Jean Mamert et Laurent Chazal, futur préfet du territoire de Belfort, ainsi qu’un chef-adjoint de cabinet, Michel Sans.
À leurs côtés sont nommés deux conseillers techniques (Maurice Pérouse et Constantin Melnik, en charge de la coordination de l’ensemble des services de police et de renseignement français), un chargé de mission pour les questions diplomatiques (Gérard Amanrich), sept chargés de mission (Jacques Dulière, Antoine Dupont-Fauville, Jean Leroy, Yves Rocca, Jacques Giscard d’Estaing, Jérôme Monod et Françoise Duléry) et un chargé de mission pour les questions sociales, Jacques Decoust.
Le cabinet est complété par Joseph Bourdeau, attaché de cabinet, et Marguerite Pouzalgues, chef du secrétariat particulier.
Plusieurs modifications interviennent au sein du cabinet, avec les départs du directeur-adjoint Yves Guéna, remplacé par Christian Lobut, préfet hors-cadre, des deux chefs de cabinet, remplacés par Jean Vaudeville, sous-préfet, la nomination d’un chargé de mission pour les questions de presse, Jean Bergonier, celle d’un nouveau chargé de mission, Jean Sribier, et de deux attachés de cabinet supplémentaires, Geneviève Barbier et René de Talhouët, et enfin celle d’une attachée de presse, Arlette de la Loyère (1960).
Michel Debré adresse sa lettre de démission au Président de la République le 14 avril 1962.
